Applications will be accepted from March 15 through April 30, 2026.

ELIGIBILITY

Children/Youth are eligible for Essiantial Items based on the following criteria:
● 24 years of age or under
● Has a documented special needs or medical diagnosis from a licensed healthcare professional (e.g., physician, therapist, or specialist)
● Needs diapers because of a health condition that affects bladder or bowel control
● Needs pediatric nutritional drinks to support healthy weight gain because of low weight or low BMI for age (below the normal range on growth charts)

● Letter from the physician stating the necessity of the supplies
● Child’s Photo & ID
● Our team will confirm your referral.

Parent/Legal Guardian must fill out the application below.

REVIEW

Within 30 days of the application being submitted and the required documents being received, an application decision will be sent by email.
● If approved, eligibility will be valid for 12 months; after this period, families must submit a new application to continue receiving support.
● Approved supplies will be provided either through monthly shipment to the child or pickup at designated pharmacies, depending on availability and family preference.
